The City of Evanston is encouraging parents of 12- to 15-year-olds to fill out the city’s vaccination contact form so the youngsters can be vaccinated.
The federal Food and Drug Administration on Monday approved use of the Pfizer vaccine for children ages 12 to 15, and an advisory panel of the Centers for Disease Control followed suit today, the final step needed before inoculations could be offered to this age group.
Completing the form for every member of your household, no matter their age, will ensure that the City can contact them as soon as the vaccine is available.
