Evanston Skokie School District 65 officials say they’ve had to reschedule some in-school vaccination clinics because the supply of H1N1 flu vaccine hasn’t yet been delivered to the Evanston’s health department.

The revised schedule is shown below.

School officials are reminding parents and guardians to complete and return their children’s consent form to your child’s school so they can participate in the free vaccination program.

Here’s the revised schedule, with new dates shown in bold.
